QA/MDS Nurse (LVN) - Lakeport Post Acute

Location: Lakeport, CA
Pay: Starting at $42 per hour, DOE
Schedule: PRN

Responsibilities

• Coordinate and oversee MDS (Minimum Data Set) assessments and submissions
• Ensure compliance with state and federal regulatory requirements
• Collaborate with the interdisciplinary team to support accurate care planning
• Monitor and evaluate quality indicators and assist with QAPI initiatives
• Provide education and guidance to staff on documentation and quality assurance processes
• Assist in audits, surveys, and care plan meetings

Qualifications

• Active California LVN license required
• Prior MDS/QA experience in a skilled nursing facility strongly preferred
• Strong knowledge of state and federal regulations related to MDS and QA
• Excellent assessment, documentation, and organizational skills
• Team-oriented, with strong communication and leadership abilities
