Living Donor Recipient Navigator (Finance)



Work Schedule:

40 hours per week, day shift. Monday - Friday 8:00AM - 5:00PM. You will work at University Hospital in Madison, WI. This is a hybrid position, and you will likely need to be onsite 2-5 days per week depending on case load, patient and clinical need. Hours may vary based on the operational needs of the department.

We are seeking a Living Donor Recipient Navigator to:

  • Provide living donor education, support, and navigation services for transplant recipient candidates.
  • The primary patient population of interest will be kidney and liver transplant recipients who would benefit from living donation and may not have immediately identified potential living donors.
  • Work with kidney transplant and living donor coordinators to Identify potential transplant candidates and educate them and their support on the importance and benefits of living donation.
  • Educate transplant candidates and their support on how to find potential living donors, including networking, and creating social media campaigns among other venues.
  • Assist transplant recipients in their journey to transplant as it relates to living donation and be available to answer questions via phone, email and MyChart.
  • Responsible for further discussions and education with transplant candidates via teleconferencing.
  • Work with the Transplant Center staff, including the Living Donor Community Education Coordinator to host educational events and presentations on the importance of living donation.

Qualifications

  • Bachelor's Degree in Social Work, Counseling, Nursing, Health Education or related field Required
  • Master's Degree in Social Work, Counseling, Nursing, Health Education or related field Preferred

Work Experience

  • 1 year experience working with transplant patient or living donors Required.
  • Social worker with transplant experience Preferred.
  • Experience living or working in a rural community Preferred.
