Hospital Liaison (Healthcare)
Westminster Towers is a large upscale Active Living Community and Health Center located just south of downtown Orlando. We are a church related, not-for-profit community service organization dedicated and committed to providing services for older adults and persons with special needs by creating and administering excellent residential and health care facilities.
Job Description:
Performance of clinical assessments for potential and current Westminster Services health center residents. Negotiation for residents who are clients of managed care organizations. Liaison for community relations, discharge planners, and Physicians. Support for all Marketing efforts. Maximize cost effectiveness across the entire continuum of care; improve customer satisfaction and enhance quality of care. Working with residents being discharged from the hospital and the hospital discharge planners. Marketing to visiting physicians and other referral sources in the community.
ESSENTIAL QUALIFICATIONS:
- Must be a licensed RN in the state of Florida.
- Certification in Case Management or Geriatrics desirable.
- Must be CPR Certified.
- Two years in long term care, including working knowledge of Medicare and Managed care protocols.
- One year in post acute rehabilitation.
- Knowledge of case management purpose and process.
- Demonstrated clinical assessment skills.
