Nurse Care Manager (Healthcare)



Under the direction of the Supervisor of Integrated Health Services, the Nurse Care Manager provides culturally competent and trauma-informed services to consumers, supporting them in designing and implementing individualized, goal-oriented strategies for care. This role is responsible for assessment, planning, advocacy, linkage, and coordination through the person-centered planning process.
The Nurse Care Manager plays a key role in ensuring comprehensive care by collaborating across systems and departments to meet consumer needs. In addition, this position supports the delivery of LifeWays Substance Use Disorder Health Home (SUDHH) model, promoting recovery, independence, and whole-person wellness while embodying LifeWays mission, vision, and values.
Key Responsibilities

Welcome individuals with empathy and provide non-judgmental, trauma-informed support.

Assess needs, develop person-centered care plans, and monitor progress.

Coordinate referrals, care transitions, and services within and outside the provider network.

Collaborate with psychiatrists, physicians, and community providers to ensure whole-person care.

Facilitate consumer participation in treatment planning and goal setting.

Support crisis planning, transition planning, and aftercare services.

Perform care management functions within the Substance Use Disorder Health Home (SUDHH) model.

Maintain accurate documentation and ensure compliance with medical necessity requirements.

Advocate for consumers to reduce barriers and promote recovery and independence.
