Located in the scenic foothills of San Bernardino, Waterman Canyon Post-Acute is a 166-bed Medicare-certified skilled nursing and short-term rehabilitation facility. We specialize in personalized care, offering services such as 24-hour skilled nursing, complex wound care, long-term care, and hospice services. Our expansive therapy gym and compassionate team of licensed professionals are dedicated to helping residents recover quickly and comfortably in a warm, welcoming environment.
💼 What You'll Do
- Deliver individualized physical therapy services to post-acute and long-term care residents
- Collaborate with a multidisciplinary team to develop and implement care plans
- Help patients improve mobility, strength, and overall physical function
- Document progress and maintain compliance with regulatory standards
- Educate patients and families on therapy goals and home exercises
- Licensed Physical Therapist in California
- Strong communication and teamwork skills
- Passion for working with the senior population
- Commitment to excellence and patient-centered care
- $48-$52/hour, depending on Per Diem or Full-Time status
