Job Title: Licensed Vocational Nurse (LVN) - Full-Time
Location: Visalia Post Acute - Visalia, CA
Shifts: AM or NOC | 12-Hour Shifts
Starting Pay: $30/hour

Key Responsibilities
Clinical Care & Supervision:
- Direct and monitor the day-to-day functions of nursing assistants
- Ensure quality care in accordance with resident care plans
- Admit, transfer, and discharge residents as needed
- Administer medications and treatments as prescribed
- Monitor and report changes in residents' conditions
Documentation & Reporting:
- Accurately complete and maintain residents' medical records
- Record physician orders, chart treatments, and update care plans
- Prepare required forms, evaluations, and reports
Leadership & Communication:
- Provide guidance and oversight to CNA staff
- Conduct rounds to ensure quality of care and staff compliance
- Communicate with residents, families, physicians, and interdisciplinary teams
- Participate in staff meetings, in-services, and training sessions
Compliance & Safety:
- Ensure adherence to infection control, safety, and facility protocols
- Participate in audits, surveys, and quality assurance programs
- Maintain confidentiality of resident and facility information
- Current and valid California LVN license
- CPR certification required
- Graduate of an accredited LVN program
- Knowledge of Medicare/Medi-Cal regulations preferred
- Experience with PointClickCare (PCC) preferred
- Ability to work independently and manage multiple priorities
- Strong written and verbal communication skills
- Must be able to lift up to 25 lbs and perform physical job duties as required
