Social Services Director (On Call/Part-Time) - Antelope Valley Care Center (Personal Services)

Pay Rate: $80/hr

• Establishes and implements care plans by assessing residents' social, emotional, and family needs; sets goals with residents and families to promote independence and well-being.
• Provides medically related social services to help residents attain or maintain the highest practicable physical, mental, and psychosocial health.
• Offers emotional support, counseling, and assistance with coping related to disabilities, health concerns, grief, and loss.
• Assists residents with healthcare decisions, discharge planning, and coordination with outside agencies for services, referrals, and resources.
• Coordinates ancillary services such as optometry, podiatry, dentistry, and psychiatric appointments.
• Helps residents with personal needs such as shopping, belongings inventory, and resolving issues of lost/stolen items.
• Secures prosthetics and assistive devices to support resident care and quality of life.
• Maintains documentation of resident social service status and participates in interdisciplinary team meetings.
• Builds strong working relationships with facility departments and community agencies to ensure comprehensive support.
• Provides in-service training to staff, promoting understanding of resident social service needs and effective interventions.
